Description: A cross pattée imposed on a wreath of laurel, with the Royal crown in a circular panel in the centre. Suspended by a ring from a plain suspension bar.

As part of the decision to remove distinctions of rank in awards for bravery, this decoration replaced the DSO for gallantry as well as the Conspicuous Gallantry Medal and the Distinguished Conduct Medal. It was first awarded in 1995 to Corporal Wayne Mills of the Duke of Wellington’s Regiment and in 1996 to Colour Sergeant Peter Humphreys of the Royal Welch Fusiliers, both for gallantry in action during service with the UN Peacekeeping Forces in Bosnia. Two awards were made in respect of gallantry by members of the SAS and SBS in Sierra Leone in May–June 2000. A further two awards were made to members of the SAS and two awards to members of the SBS in Afghanistan in October 2001–March 2002, while awards to Justin Thomas, RM, and Lance Corporal
Michael Flynn, Blues and Royals, both for gallantry in Iraq, were gazetted in October 2003, and that to Sqdn Ldr I. J. McKechnie, Royal Air Force, was gazetted on 9 September 2005, making a total of 11 in the first decade. Since then other awards have been made taking the total number of awards to date to 60, including a collective award to the Royal Irish Regiment which appeared in the LG Supplement, December 18, 2006 and three posthumous awards.
